cdc-logo.pngThe Centers for Disease Control is offering a data-collection tool that gives users a way to develop an action plan; by evaluating the health of their community from top to bottom. The CHANGE tool helps define and prioritize possible areas of improvement. In September and October volunteers will fan-out across the city to research everything from stair-use signage to tobacco policy. Having this information as a guide, community team members can create sustainable, community-based improvements that address the root causes of chronic diseases and related risk factors.
